Sign informed consent on a phone
A clinic may ask you to sign informed consent on your phone during care. This is not booking. You need a patient account.
The scan opens this screen. Patients use the first button:

Patient phones sign in to the Patient Portal. A provider phone captures a supervised signature and requires attestation. This does not authenticate the patient.
Before you scan
Section titled “Before you scan”- Use the phone in front of the clinic staff.
- Have the email you use for the Patient Portal.
- The code expires in 15 minutes. Ask for a new code if it expires.
- Scan the one QR the clinic shows, or open the copied link they give you.
- Choose Patient Portal phone.
- Sign in to the Patient Portal if the product asks.
- Read the consent text.
- Draw your signature and save.
- You can close this page. Hand the phone back. Staff watch the desktop until it shows the request is signed.
If something fails
Section titled “If something fails”- Wrong button. Ask the clinic to generate a new code and scan again. The first code is no longer usable after that screen.
- Code expired or already used. Same: a new code.
- Sign-in loop. You must finish Patient Portal sign-in. This is not the Independent Portal and not an IPS staff sign-in.
